How to Enroll

Download the Student Enrollment Packet and supplementing documentation:

Individual Forms:

  • Student Enrollment Checklist
  • Student Enrollment Application
  • Health Check Form (if applicable)
  • Health Check Waiver (if applicable)
  • Dental Check Form (if applicable)
  • Optional Survey
  • State Immunization Form

    After you have filled out this packet, please mail to the Administrative Office:

    Once all paperwork is submitted, you will be contacted by your Educational Facilitator who will set a time with you to complete the enrollment which includes:

    Responsibilites and Limitations

    By choosing to enroll their children in Dehesa Charter School, parents accept co-responsibility for their children's education (please read our program description).

    Also note that, in accordance with California State Assembly Bill 544, it is not legal for a student to be concurrently enrolled full time in a charter school while in any other school, public or private.
